Do you ever use dosing beyond 20mg of Cortef?
I have your book but can't find info on the following: Do you ever have patients go beyond 20mg of Cortef? Do you have patients slowly ramp up with hydrocortisol, or do you prescribe a full dosage right from the start?

I rarely go over 20 mg , though there are times I will. I most often start low and work up, but the adjustments to the dosing are often made quickly--but it is on a case by case basis.
