Bioidentical Hormone Replacement

Published: August 11, 2012

In previous articles I discussed the hormone-generating adrenal and thyroid glands. In this article I focus on a hormone issue specific to women. A woman's ovaries produce two key hormones in the body: estrogen and progesterone. But as women age, their ovaries' ability to produce these hormones begins to drop - with unwanted consequences. Fortunately, this problem can be addressed through bioidentical hormone therapy.

Dizziness in CFS: What You Need to Know About NMH, POTS, and Adrenal Issues

Welcome to part 5 of my series reviewing and updating the core elements of the SHINE protocol for CFS and fibromyalgia. In the first 4 parts of this series, I focused on Sleep — the "S" in SHINE. This week I begin my discussion on Hormones — the "H" in SHINE. And in particular, adrenal hormones. Adrenal glands sit like caps on top of your kidneys. The middle part of the gland makes adrenaline, while the outer part makes cortisol, DHEA-S, aldosterone, estrogen and testosterone. About two-thirds of chronic fatigue patients appear to have underactive adrenal function.

Welcome to part 4 of my series reviewing and updating the core elements of the SHINE protocol for CFS and fibromyalgia. Just as I did in the first 3 parts of this series, in part 4 I again focus on sleep. This time I look at a pattern that bedevils the nights of most patients with CFS and fibromyalgia — waking up between 2 and 4 a.m. in the morning and being unable to go back to sleep.

In the first two parts of my multi-part series reviewing the core elements of the SHINE protocol for CFS and fibromyalgia — Sleep, Hormones, Infections, Nutrition and Exercise — I talked about sleep problems that are common in CFS/FM and how you can address them using natural remedies, better sleep hygiene, and multiple, low-dose sleep medications. In part 3, I continue my discussion on sleep with a special focus on four specific sleep-related disorders that are frequently found in people with CFS/FM.

In this multi-part series, I am reviewing the elements of the SHINE protocol for CFS and fibromyalgia: Sleep, Hormones, Infections, Nutritional Supplements and Exercise — I'm reviewing the basics and also discussing new developments in therapy for each of the five elements. In part 1, I looked at natural remedies for sleep. In part 2, below, I continue talking about sleep, first looking at "sleep hygiene" (daily habits that can help or harm sleep), and then at how to effectively use multiple, low-dose medications to address the sleep problems that are so common in CFS/FM.

Natural Remedies for Deeper Sleep

In the previous two newsletters, I discussed what people with CFS/FM should do "when all else fails" — what to do when the SHINE protocol (Sleep, Hormones, Infections, Nutritional Supplements, Exercise) doesn't work, or doesn't work as well as you'd like it to. Of course, I'm happy to say that our research shows SHINE usually does work, helping more than 85% of CFS and fibromyalgia patients feel significantly better after just 3 months. And that's why I'm going to devote the next series of newsletters to a review of key elements of SHINE. Because it's always good to get back to the basics — to the foundational self-care and medical care regimen that can relieve pain, restore energy and counter the other symptoms of CFS/FM — by addressing the underlying causes of this condition.

Vitamin D and Chronic Widespread Pain (e.g., Fibromyalgia)

Published: August 10, 2012

Vitamin D deficiency has been found to be very common in chronic pain. This study looked at vitamin D levels in 6,824 white British adults who were 45 years old. Chronic Widespread Pain (CWP) was present in 14.6% of those with a vitamin D level (25 OHD) under < 25 nmol/l, 14.7% for a vitamin D level of 25-49 nmol/l, 11.5% for 50-74 nmo/l, 7.7% for 75-99 nmol/l, and 9.7% for participants with > 100 nmol/l. That means those with a low vitamin D level had almost twice the risk of having widespread pain (and likely also fibromyalgia). This connection was only found in women (3/4 of those with fibromyalgia are women).

Good news from the 2012 annual meeting of the American Academy of Pain: A study from researchers at Stanford University School of Medicine showed that a low dose of the drug naltrexone significantly reduced pain in patients with fibromyalgia. The study confirms earlier, similar findings from the same researchers, published in Pain Medicine in 2009.
